Authorized Signature & Certification Forms
Each district must have an up to date Authorized Signature Form on file. If the district wishes to designate additional authorized designees for certifying data submissions, they must submit a new Authorized Signature Form.
The district Reports Coordinator is responsible for completing an Authorized Signature Form whenever a new superintendent arrives, or the authorized designee is no longer the designated alternate signature on the form. The Reports Coordinator can NOT be the authorized designee. The following PDF form is available to download for districts to appoint authorized designees for database certifications.
District superintendents shall certify that workforce education enrollment and performance data used for funding allocations to districts is accurate and complete in accordance with reporting timelines established by the Department of Education. The district Superintendent, authorized designee who is on file, or the Charter Board director must sign the End of Year Data Certification Form. The following PDF form is available for certifying submitted data at the end of the WDIS reporting year.
