2017-2018 End-of-Year Stakeholders’ Surveys
Student, Teacher, and Adult Family Member

At the end of each year, 21st CCLC programs must survey participating students and their school-day teacher and adult family member. This is a requirement of the funds that support the 21st CCLC program.

You are being asked to provide information about your perception and impacts of the 21st CCLC program on student academic and personal development. We assure you that there are no ‘right’ or ‘wrong’ answers and no identifying information will link you to the survey. The information obtained will assist the 21st CCLC program in making improvements to better serve its students and families in the future.

Surveys must be completed by Tuesday, May 15, 2018. The survey links for electronic surveys will be closed after this date.

Below are the links for the 21st CCLC surveys. Select the link that best describes you (student, school-day teacher, or adult family member). When completing these surveys, please think about your experience with the program this program year.

Before you begin the survey, please make sure that you know the appropriate 21st CCLC afterschool program and center name. This information should be provided to you by the program’s director/site coordinator.

Adult Family Member of a Student Participating in the 21st CCLC Program

These surveys are available in English, Spanish, and Creole. Adult family members may choose to complete the survey in paper format (English and Spanish) and mail it directly to the survey manager. If you would like to use the paper version, please ask the program staff for the paper survey and a postage-paid envelope.
