Promissory Note
By completing this Promissory Note and submitting it to the Florida Department of Education, Dale Hickam Excellent Teaching Program, you hereby agree to the following:
- To complete in the NBPTS certification process for the 2007-2008 application year and to adhere to all program guidelines and timelines; or, to completely withdraw from the process by the withdrawal deadline date of March 31, 2008.
- To pay online the non-refundable $65 application processing fee and the non-refundable portion of the initial fee ($250) that I am responsible for paying;
- In the event of my withdrawal from the process, to pay all applicable withdrawal fees ($250) and all reasonable attorney’s fees for trials and all appeals and other costs and charges that are necessary for the collection of any unpaid fees;
- To submit a completed portfolio by the portfolio deadline of March 31, 2008, and to complete all assessment exercises by June 15, 2008;
- To remain a full-time classroom teacher in a Florida public school for the year immediately following completion of the program whether or not the NBPTS certification is obtained.
