2010-2011 Department of Juvenile Justice Student Exit Assessment
- Submit this record during reporting period 5 for all PK – 12 students
who exited a Department of Juvenile Justice (DJJ) commitment, day treatment,
or early delinquency intervention program between July 1 and June 30 of the
reporting year and who have Basic Achievement Skills Inventory exit assessment
test scores. More than one record should be submitted if a student exits more
than one DJJ school during the year. If two exit assessments are given in the
same subject area at the same school in one school year, only the most recent
assessment should be submitted.
- Report exit test information for each of three subject areas (reading, mathematics, and language arts).
- DISTRICT NUMBER, WHERE TESTED: Enter the district number the student was enrolled in at the time of the assessment.
- SCHOOL NUMBER, WHERE TESTED: Enter the DJJ school number the student attended at the time of the assessment.
- TEST SCORE: Enter the test score, right justified, for each appropriate subject area using leading zeroes.. For each subject area tested, enter the Growth Scale Value that the student earned. Growth Scale Values range from 0035 - 0166.
- TEST FORM: Enter the test form, A, B or Z (test not administered or not applicable) that was administered to the student.
- TEST LEVEL: Enter the test level, left justified, that was administered to the student. Test Level may be 1, 2, 3, 4 or 0 (no test administered or not applicable).
- ERROR CODES: This field is used by the Department to report to districts the specific errors found in the record during the state edit process. This field should contain filler (spaces, blanks) when the record is transmitted to the Department.
- KEY FIELDS: The key fields for this format are item numbers 1, 2, 3, 4 and 5.
