2003-2004 Student Dropout/Match Information
1. This is a voluntary format used to submit information for students identified as either habitual truants or dropouts to determine whether they are currently enrolled in a Florida Public School.
2. For reporting periods 2, 3 and 5, submit this record for each student (PK-12) who has been withdrawn from the school, since the first day of the school year, and for whom any one of the withdrawal codes that would identify the student as a dropout or habitual truant has been applied.
