E-Rate 2005

Thank you to all that provided information to help make this information accurate. Site updated on January 15, 2004 (this is an update from the November 2003 initial release); previous data and guidance is no longer valid when filing for E-rate discounts in Year 7; (2004-2005) i.e. this is the most accurate information we have from the DOE databases.

The following information is provided from the Florida Department of Education database, which was collected from the Food Service sections of the school districts. It is data that follows the National School Lunch Program (NSLP) policy. The Free and Reduced Lunch Program data contained within the Form 4A datasheets on this web page is derived from the NSLP data.

As with last year (Year 6), worksheets 4A of E-rate Form 471 are available for each Florida school district. OET staff completed the Excel worksheets using data from last year's school year survey 3 (2003-2004). If you use these 4A sheets, please review and ensure completeness so that your Form 471 application does not have Minimum Processing Standard errors that would have your application returned unprocessed and thereby potentially lose E-rate funding.

In addition, please review the worksheets for your respective district, noting that, in some cases, school "entity numbers" might be missing. Entity numbers may be missing due to: the schools' entity numbers were not collected last year; the school/site name has changed; or the school/site is a new facility. The schools' entity numbers are required to complete the application process for Year 5 of E-rate. A number of school districts responded to our e-mail requesting review but not all. So it is important that a review of the 4A is conducted prior to using it for your Form 471.

If a school's entity number is missing for one or more of your schools/sites, please obtain the needed entity number from the SLD and send this information to Maureen Githens at Maureen.Githens@fldoe.org; this information will be used to assist Melinda Crowley with FIRN's E-rate application.

Also, the Excel worksheet may not contain all of the schools/sites your district claims for E-rate purposes, but the worksheet does need to contain the school/site information that FIRN claims for E-rate. If you feel OET has not adequately represented all of the schools/sites in your district, please contact Maureen. Remember that we are using last year’s data, so NEW schools as of this school year will not be on the list. All schools on the 4A need to be on FIRN’s network. If they are not, then they should not be on the 4A on this website. This does not preclude a school district from putting them in their own application for eligible E-rate services.

To download the worksheets, click on your district name below. You will be prompted for a user name, which is the name of your county (not case sensitive) and also for a password. The password is your county name spelled backwards; the password is not case sensitive. If you experience technical difficulties, please contact Tony Giordano at Tony.Giordano@fldoe.org.

If you still have any other changes/deletions/corrections, please submit the completed worksheets, with the entity number included immediately to Maureen.
