Please Note: The following information is provided from the Florida Department of Education's database, which was collected from the Food Service sections of the school districts during the annually scheduled surveys of the school districts' student information. It is data that follows the National School Lunch Program (NSLP) policy. As with previous years, the Bureau of Educational Technology uses the school-level FRL reports collected and processed by the Bureau of Education Information and Accountability Services (EIAS) during the Survey 2 period, conducted in October, to complete the Block 4 Discount Calculation Worksheets of E-Rate Form 471 displayed on this website. These Block 4 Discount Calculation Worksheets of E-Rate Form 471 are available for each Florida school district to use, should they choose to do so. School districts may alternatively decide to use its own data from reports of FRL submitted to USDA.
Please review the 2014 worksheets for completeness! They use the 2013-2014 Survey 2 FRL data as of 2/3/2014.
If you use these worksheets, please review and ensure completeness, so that your Form 471 application does not have Minimum Processing Standard errors that would have your E-Rate application returned unprocessed and thereby potentially lose E-Rate funding. Please Note the following pertaining to the new Community Eligibility Provision (CEP), as recorded in a February 11, 2014 email from the Program Integrity Assurance team of the Universal Service Administrative Company (USAC):
'We understand that for the school year beginning July 1, 2014, the Community Eligibility Provision (CEP), formerly known as the Community Eligibility Option (CEO), or Provision 4 of the National School Lunch Program (NSLP) will be available nationwide to all eligible LEAs and schools. We would like to remind you that CEP is not an approved method for calculating E-rate discounts. Therefore, CEP data is not an acceptable source for E-Rate discount validation. If your state has schools participating in CEP for the first time in 2013, please provide the Fall 2012 free and reduced lunch information. If a school has participated in CEP for multiple years, please provide the free and reduced lunch information for the most recent year when the school participated in the non-CEP provisions of the NSLP. If you have a new school that has only ever participated in the NSLP through CEP, and which therefore does not have any traditional NSLP data, you may use the shared school district average for that school.'
The Final 2012-2013 Free/Reduced Price Lunch Eligibility by School, Survey 2 data as of 05/08/2013, and validated by the Florida Department of Education, is located at the Bureau of Education Information and Accountability Services' Data Publications and Reports: Students website. You may also use the 2013 Block 4 Discount Calculation Worksheets of E-Rate Form 471 displayed on this website which was compiled using 2012 FRL Survey 2 data.
