E-Rate Worksheets


Please Note:

The Florida Department of Education, Bureau of Educational Technology, will post school district and National School Lunch Program (NSLP) on January 31, 2015 for the district to use for creating the FCC Form 471 Block 4 Worksheets. Please refer back to this website after January 31, 2015 for the 2015 datafile.

 Due to the recent Federal Communications Commission (FCC) E-rate Modernization Order (PDF) adopted on July 11, 2014, there are significant changes to the new FCC Form 471 (PDF), one of which is that the Form 471 must be filed electronically. Please refer to the FCC Form 471 Instructions for further information. Because of these changes, the Bureau of Educational Technology will no longer create and post the FCC Form 471 Block 4 worksheets as we have done in past years.

Beginning with the 2015 E-Rate application year, the Bureau of Educational Technology will post on this website school district information collected in the Department of Education's Master Public School database. These are active public schools and charter schools recognized by the the Department of Education as elementary or a secondary school as defined in the No Child Left Behind Act of 2001 (20 U.S.C. Section 7801 (18) and (38)) which is not operating as a for-profit business, and does not have an endowment exceeding $50 million. 

For the E-Rate application process, beginning in 2015 and each year thereafter, the Florida Department of Education will post on this website the National School Lunch Program (NSLP) Free and Reduced Lunch (FRL) Eligibility data collected by the Division of Food, Nutrition and Wellness from the Florida Department of Agriculture and Consumer Services (DFNW-FDACS) for schools that have submitted an application to the DFNW-FDACS to participate in the NSLP. The FDACS - NSLP October Data (XLS) represents all of the approved NSLP sites participating as of October 31, 2014.

Districts can use the DFNW-FDACS data for the Form 471 Block 4 School District Calculation. E-Rate applicants should refer to the Universal Service Administrative Company's (USAC) Calculating Discounts website for further information.
