National School Lunch and Breakfast Program
How To Apply
To apply for the National School Lunch Program and School Breakfast Program there are three steps that must be followed:
Step 1:
New sponsors of the National School Lunch Program and School Breakfast Program must register and attend a School Lunch training, as well as a NSLP Web training.
Step 2:
Choose which programs your facility would like to administer, and indicate it on your Food Service Agreement. For more information regarding the different programs, visit Program Description and Requirements.

- Food Service Agreement PDF Word (2 originals must be submitted)
- Sponsor Application Process Agreement
- Free and Reduce-Price Policy Statement(s) (2 originals must be submitted)
- Civil Rights Pre-Award Compliance PDF Word
- Certification Regarding Lobbying
- Certification Regarding Debarment
- CNP Web/Common Logon Security Agreement
- CNP Web/Common Logon Change or Delete Form
- For Private Organizations Only: Copy of tax-exemption 501(c)(3) letter from the IRS (Application for Recognition of Exemption)
- For Residential Facilities Only: Copy of current license for each site(s) participating in the program
- Wellness Policy
- Food Safety Plan
- Meal Counting and Claiming Procedures
- 2 Weeks Menu per Meal Service
- Caterer/Vendor Contract see Contracting for Meal Service)
- Food Service Management Company Contract/Addendum/Renewal see Contracting for Meal Service)
- Parent Letter (form is only required if changes are to be made to existing format)
- Application for Free and Reduced-Price Meals (form is only required if changes are to be made to existing format)
- Notification Letter to Parents (form is only required if changes are to be made to existing format)
- Production Record (form required if not using prototype)
- Edit Check (form required if not using prototype)
- Verification Letter (form required if not using prototype)
- Letter of Eligibility Change (form required if not using prototype)
If Applicable:
Step 3:
Sponsors must submit an On-line Application for the National School Lunch Program, School Breakfast Program, and After School Care Snack Program via the CNP Web Program.
The following items must be submitted via the CNP Web Program:
- Application Information Page
- Site Information Page
- Checklist Page
Once your online application is complete and the Food Service Agreement is signed by both parties (the Florida Department of Education and you, the sponsoring agency), the CNP Office will mail one copy of the signed Food Service Agreement to the School Food Authority Contact listed on the application.
Once your application has been reviewed and approved, you will receive an Online Confirmation (via the CNP Web Program). You may then begin claiming reimbursable meals served to the students.
